Mandatory Documents for MBBS and BDS Admissions in All Govt Institutions of Punjab

Here you will know all details about the mandatory documents required for MBBS / BDS admission in all Government institutions of Punjab. The admission process for MBBS and BDS in Government Medical & Dental institutions will begin after the declaration of the official result of the MDCAT test 2022. Below listed documents are mandatory along with the application form of the relevant University for admission in MBBS and BDS. We summarize the documents list which is required to attach with the application.

Matriculation certificate or result card issued from concerned BISE / equivalent certificate of O Level etc issued by IBCC.

HSSC certification or result card issued by concerned BISE / equivalent certificate A-level issued by IBCC.The candidates who have foreign qualifications shall also upload their original results cards by the foreign University verifying their subjects of study( Biology, Chemistry, Physics/Maths.

Domicile certificate of the candidates. domicile of Punjab is mandatory for the aforementioned seats, however, candidates of ICT domicile can apply only for open merit seats.

In the case of Overseas Pakistanis, the Punjab domicile of the father/mother is also acceptable if candidates are born out of Pakistan.

valid CNIC/POC/B-form/Passport of the candidate.

Recent color photograph of the candidate with a white or sky-blue background

Disability certificate issued by the National Commission for Rehabilitation of special people of Pakistan(NCRDP). Ministry of social welfare or provincial or district social welfare department( for students with disabilities only) school leaving certificate issued by the heads/principals of concerned educational institutes of the underdeveloped districts verifying that students did the study from One to Twelve in the same district. Candidates applying for underdeveloped district seats should also upload the form( template given on the UHS website) verified by the DEO/CEO Education and DC of the relevant district.

Original Certificate issued by Cholistan Development authority stating that candidate is Cholistani by birth and his residence is Cholistan (for candidates applying for cCholistan seats).

valid stay card/ residence card/ green card/ Iqama or similar documents issued by the country of residence along with a valid Pakistan passport (candidates applying on overseas Pakistanis seats)

Hifz Quran certificate issued by a registered Madarassa ( for candidates claiming to be Huffaz Quran)
specimen signature and left thumb impression LTI of the candidate in blue/black ink on a white paper
